I give consent to the collecting, storing and processing of my above stated personal data by company EF Mobility s.r.o., seated in Olbrachtova 2006/9, 140 00 Praha 4 – KRČ, reg. no.: 25080229, registered in Commercial Register run by Municipal Court in Prague, section C, insertion 47847 (hereafter “P Automobil Import”) as controller and to third parties designated by P Automobil Import as processors (particularly to the members of distribution network of P Automobil Import) for the purpose established below. The purpose of processing the personal data is their use for further commercial and marketing activity, particularly for market survey, statistical purposes, monitoring sales data, providing after-sales services and dissemination of commercial communication in the sense of § 7 Act No. 480/2004 Coll.,on certain information society services, as amended. I confirm, that in accordance with the Act No. 101/2000 Coll., on the protection of personal data, as amended (hereafter “Act”), I am informed about the right of access to my personal data and about other rights stated particularly in § 21 Act including the right to request in writing providing information about my personal data processed by P Automobil Import and providing explanation if I believe that my personal data are being processed contrary to the Act; I am aware of the right of removal the situation thus incurred, blocking the inaccurate personal data, their rectification, completion or liquidation etc. All rights may be exercised in writing at the address of P Automobil Import. I give this consent for a period of 5 (five) years from the date of the consent. I was acquainted with all the filled parts of this form, all data are accurate and truthful and are given voluntarily, I confirm this consent by ticking the box.
